To attend any activity from Bicycle Revival you must have filled in the below waiver before arrival. Failure to complete the below will mean you cannot join the activity, and there will be no opportunity to complete onsite unless you have a mobile device with internet access.

By joining a Bicycle Revival ride or activity, I understand that cycling and physical activity come with certain risks. I accept that there is a possibility of injury and in rare cases, serious injury or loss of life, as a result of participating in any Bicycle Revival activities. I choose to take part voluntarily and accept full responsibility for my own safety and wellbeing while participating.
I acknowledge that cycling, especially on public roads, involves potential hazards. I agree to use a roadworthy bicycle, wear an appropriate cycling helmet, and wear suitable clothing. During winter or low-light rides, I will carry and use front and rear lights to ensure I’m visible to others and to help keep the group safe.
I understand that Bicycle Revival provides third-party insurance for participants while taking part in official group rides or events. This insurance covers liability to others but does not provide personal accident or property insurance.
By completing this form, I confirm that I have read, understood, and accept these risks, and that I’m participating at my own risk.
